Most of the items listed under ‘deductions/tax’ are taxes required to be withheld from your pay.
FED = Federal income tax
SS = Social Security tax
MED = Medicare tax
OH = Ohio state tax
NONE = No tax location
NAPCIT = City tax withholding for employee’s residence or work site
3504 = School income tax is listed under the school district’s number
MMSINGLE = Medical Mutual Single Health Insurance Premium
MMSPOUSE = Medical Mutual Employee/Spouse Health Insurance Premium
MMFAMILY = Medical Mutual Family Health Insurance Premium
MMCHILD = Medical Mutual Employee/Child Health Insurance Premium
